Folic acid fortification

Mixing a small, mandated dose of folic acid into flour and grain products has prevented an estimated 1,326 neural tube defect-affected births a year in the United States since 1998.

1,326 lives — neural tube defect-affected births averted per year in the United States by mandatory folic acid fortification, based on surveillance data from 1999 to 2011

A CDC surveillance-based estimate for the United States only (95% confidence interval 1,122 to 1,531), not a global count. The same fortification approach has since spread to other countries.

How it saves lives

Folic acid, the synthetic form of vitamin B9, is needed in the first weeks of pregnancy to help the neural tube close properly into a working brain and spinal cord. Because many pregnancies are unplanned, and the tube closes before most women know they are pregnant, relying on supplements alone misses many cases. Adding a small, standard dose to flour and cereal grains reaches nearly everyone automatically, without changing anyone's diet.

The story

For decades, neural tube defects were treated as a matter of chance. That changed once a trial tied the condition to a shortage of one B vitamin, taken before most women even know they are pregnant. The United States mandated adding folic acid to enriched grain products at the start of 1998, and birth defects registries recorded the shift almost immediately. Surveillance data gathered from 19 state and city programs over the following decade put a number on what changed: about 1,326 births a year that would otherwise have been affected by a neural tube defect were not.

From the record

The number of births occurring annually without NTDs that would otherwise have been affected is approximately 1,326 (95% confidence interval = 1,122-1,531).

Asked often

How many births has folic acid fortification saved?

CDC surveillance from 19 US birth defects programs found that mandatory folic acid fortification, in place since 1998, prevents an estimated 1,326 neural tube defect-affected births a year, with a 95% confidence interval of 1,122 to 1,531.

Who discovered that folic acid prevents birth defects?

A UK Medical Research Council trial led by Nicholas Wald in 1991 established that folic acid taken before conception sharply cuts the risk of neural tube defects, which led the United States to mandate fortifying grain products with it in 1998.
